During your first Home Health visit, a member of our care team will come to your home to evaluate your health needs and begin developing a personalized care plan. The visit typically includes reviewing your medical history, medications, and current health concerns.

Your Home Health provider may check vital signs, assess mobility or care needs, and discuss any therapies or services recommended by your doctor. This visit also provides an opportunity for you and your family to ask questions and learn more about the care you will receive at home.
