  • Home Health care provides medical services to patients in the comfort of their own homes. Services may include skilled nursing care, physical therapy, occupational therapy, and speech therapy. The goal of Home Health is to help patients recover from illness or injury while receiving care in a familiar environment rather than in a hospital or clinical setting.

    Home Health can be especially beneficial for patients who have difficulty traveling to a doctor’s office or medical facility. By receiving care at home, patients can conserve energy and avoid transportation challenges that may be costly or difficult for some individuals.

    Our Home Health nurses provide personalized, one-on-one care for a wide range of patients, including older adults, individuals with disabilities, those recovering from surgery or injury, and patients managing chronic conditions. Services may include monitoring vital signs, medication management, wound care, mobility support, and patient education to help individuals safely manage their health at home.

    This individualized approach also allows our care team to build meaningful relationships with patients and their families, creating a level of trust and connection that supports better care and recovery.

  • Patients eligible for Home Health typically must be considered homebound and receive a referral from a healthcare provider, such as a physician, hospital, clinic, or nursing facility.

    The Hale County Home Health team proudly serves patients throughout Hale, Greene, Perry, and Marengo counties and accepts most major insurance plans, including Medicare, Blue Cross Blue Shield, Humana, United Healthcare, and Blue Advantage.

    If you or a loved one may benefit from Home Health services, speak with your healthcare provider about obtaining a referral and determining eligibility for care.

  • Home Health care is typically provided in 60-day care periods, with the option to extend or renew services depending on the patient’s medical needs and progress.

    At the end of each care period, the patient’s healthcare provider and Home Health team review the patient’s condition to determine whether continued services are necessary.

  • During your first Home Health visit, a member of our care team will come to your home to evaluate your health needs and begin developing a personalized care plan. The visit typically includes reviewing your medical history, medications, and current health concerns.

    Your Home Health provider may check vital signs, assess mobility or care needs, and discuss any therapies or services recommended by your doctor. This visit also provides an opportunity for you and your family to ask questions and learn more about the care you will receive at home.
